Your patient is a 38-year-old stay-at-home mother with a chief complaint of low back pain that worsens with stooping or forward bending that “makes the pain shoot down” her posterior Right leg to the point of her foot. She first noticed this two months ago when she was cleaning her house. A week after this she noticed she could not lift her right foot as well as her left when walking, and that her foot would sometimes go numb whenever she sat for an extended period of time. She is 5’10” tall and weighs 204 lbs, and you have observed a slightly guarded flexed posture. Lumbar spine MRI findings report abnormal posterior-lateral fluid extension into Right-sided L3-L4 and L4-L5 disc space. The PT evaluation indicates positive findings with an (R) SLR test and Slump test on physical therapy evaluation, as well as limited spine mobility and trunk weakness.

Spine Case Study #2 ❖

Your patient is a 50-year-old male computer programmer with a chief complaint of neck pain and accompanying headaches. He reports having a history of neck pain, especially after hours of sitting at his computer. However, after a car accident 2 weeks ago in which he suffered a mild whiplash injury, he has noticed increased pain and decreased tolerance for sitting at his work desk. He denies dizziness, nausea, any visual impairments, or difficulty with cognitive functions. He was treated by EMS at the time of the accident and diagnostics were unremarkable other than for whiplash injury. According to the evaluating PT, he presents with rounded shoulders and forward head posture in sitting.
